In addition, here is the RELEASE OF INFORMATION FORM to complete so that I may speak with relevant individuals.
For those participating in a Custody Evaluation, here is the CUSTODY EVALUATION CONTRACT FORM and CUSTODY INFORMATION FORM, along with the RELEASE OF INFORMATION FORM. The Contract Contract Form needs to be submitted to me (ideally by email) or provided at the first appointment. The Custody Information Form and Release of Information Form can be submitted later in the evaluation process.
